Custom questions let you collect service-specific details from customers before a booking or event.
Add questions
In Shopify admin, open Apps, then open Servv.
Go to Navigation -> Question templates section
Create a new question template with questions which you can attach to a booking page.
Create or edit the service connected to the booking page.
Open the questions area.
Select template created in step-3 above or add each question
Save the service.

Review responses
Use the booking or registrant details in Servv to review customer responses before the service starts.
